The forms in this section must be completed and returned no later than one week prior to your child's expected start date. The Health Inventory and Immunization form need to be completed by your child's Pediatrician. Your child must have a complete file to begin care. Refer a Friend Program
Current families who refer a new family that enrolls their child/ren and stay for at least a month are eligible for a credit of $25.00 on their tuition. Print out the referral form from the link below this paragraph. Please have your enrolling friend fill it out and return it to the office with their enrollment packet. If you have any questions about this program that we are offering, please do not hesitate to contact a member of the management team. (**This offer applies to new enrollment only.
